When contacting an instructor to request access into a class, provide them with: the course number the course CRN your 811# and UGA email address, and a link to the form If there is not an instructor listed for the course, contact the Area Chair. ONLY THE INSTRUCTOR OF A COURSE CAN SUBMIT THIS FORM. If a form is submitted by a student, it WILL NOT be processed. PLEASE NOTE: Sending the form to an instructor DOES NOT register you for class. You will need to complete the registration process in ATHENA once you have been notified that the form has been processed. Time conflict override needed?
